Please do NOT submit an offer form before speaking with an Agent regarding the Show Fee and availability. Once this form is received it is considered an official offer and will be submitted to StreetJam Television Management for approval. If accepted, a performance contract will be issued to the Purchaser and the Purchaser agrees to abide by all terms and conditions on the contract and riders. The person submitting offer sheet MUST be at least 21 years old.
